A few questions, with a purpose

Each interactive check uses an established screening instrument. The questions ask about specific experiences over a defined period. Read the instructions at the beginning and choose the response that best reflects your experience.

You can go back and change a response before viewing your results. There is no time limit, and you can stop whenever you need to. Our interactive checks are intended for adults aged 18 and older.

What the result can tell you

Your answers are scored using the rules for that instrument. Some results describe symptom severity; others identify a pattern that may be worth discussing with a professional. The number is one piece of information, not a complete picture of your health.

A screen cannot establish a diagnosis, rule out a condition, or determine which treatment is right for you. Your history, health, circumstances, and preferences belong in that conversation.

Your next step is yours to choose

After a check, you can print your result, explore support in your state, or read more. You never need a particular score to ask for help. You do not have to use a provider we link to.

Our opioid and drug-use check uses the TAPS screening tool. Our postpartum mental health page is a separate support guide with information and care resources; it does not produce a screening score.

A private space to reflect

Your answers and score are calculated in your browser and are not saved by this site. There is no account or email requirement. Refreshing or leaving the page clears the active check. Printing creates a copy on your own device or printer.

There’s no wrong time to ask for support.
